Platforms like Reddit and LinkedIn have already seen growing concern among users, with some joking that machines are taking over the internet, while others are seriously alarmed by the increasing sophistication of artificial engagement.
For those of us in medicine, particularly in the online healthcare space, this development isn’t just intriguing. It’s a wake-up call. Physicians and healthcare professionals have steadily increased their presence on social media in recent years, using platforms to share accurate medical information, connect with patients and peers, and combat misinformation. But what happens when the comment section of your carefully crafted post on cancer screening is flooded with persuasive responses from what appear to be real people.. except they’re not? What happens when a viral post touting dangerous medical advice is being liked and shared by a network of coordinated, AI-run accounts?

They don’t need sleep. And they don’t care about truth. They care about influence.
Doctors and scientists are trained to evaluate evidence, communicate clearly, and follow best practices rooted in research. But we’re not trained to compete with algorithms designed to win attention at all costs. In the current landscape, facts aren’t always enough. A well-intentioned post backed by decades of clinical research can be drowned out by an army of fake accounts, all agreeing with each other, creating the illusion of controversy or public doubt. This distorts the way health information is perceived and shared, even when the science is clear.

This is also a call to action for medical education and professional development. Digital literacy must now include an understanding of how social media functions, how algorithms promote content, and how AI-generated engagement distorts conversation. These are no longer “extra” skills. They are essential for effective communication in the modern era. Clinicians need to understand not just what to say, but how digital platforms may shape, warp, or silence that message.
At the same time, we must push for accountability and transparency from the platforms themselves. Just as we regulate medical advertising and require disclosures in clinical trials, social media companies must be held to a standard when it comes to identifying and limiting AI-generated content, especially in sensitive areas like health. Users should know when they’re interacting with a bot. There should be systems in place to flag coordinated disinformation campaigns. Otherwise, we risk losing the integrity of digital discourse entirely.
Despite all of this, there’s still hope. Authentic, human voices, especially those grounded in science and compassion, still matter. In fact, they may matter more now than ever. The more artificial noise there is online, the more powerful real, empathetic, transparent engagement becomes. When patients see a real doctor using their real name, speaking plainly and thoughtfully about real health issues, it cuts through the fog. It reminds them that there are people behind the posts. And while bots can simulate engagement, they can’t replicate human trust.
So, for those of us in medicine who’ve chosen to be present in the digital space: keep showing up. Keep posting. Keep educating. The internet may be changing, but the need for credible, compassionate health communication hasn’t gone away. If anything, it’s never been more urgent.
